OCULIFE TERMS & CONDITIONS

Join Oculife and experience premium eyecare beyond sight. 

INTRODUCTION

 

Thank you for agreeing to join the Martin Steels Emsworth Oculife and agreeing to abide by these terms. Your membership benefits, start from the date of this Agreement, subject to having had Enhanced Eye Examination within the last four weeks of the date of this Agreement. 

1.2      Oculife entitles you to: - 

1.2.1     Annual eye care as required (excludes Contact Lens Services). 

1.2.2     Emergency appointment as required. 

1.2.3    Exclusive 10% off eyewear and sub-specialized services (see 1.2.5). 

1.2.4     Children under 19 in the same household can enjoy premium private eye care (excludes Contact Lens Services) for just £1 per month per child added to your plan. Under 19 eye examination is every two years. 

1.2.5    Glaucoma Monitoring Service, BlephEx treatment, lacrimal syringing and punctum plug implants are beyond the remit of Oculife but they are available at member-only rate. 

 

TERM AND TERMINATION 

2.1      Membership benefits are available conditional upon you subscribing for a minimum term of 18 months (Minimum Term) and making timely payments. 

     2.2      Cancellation will need to be made in writing. If you cancel:- 

2.2.1      Oculife; or 

2.2.2      your direct debits  

prior to the end of the Minimum Term, you will have to repay the lower of the discounts and value of care that you have received or the balance of the Oculife instalments that remain due to the end of the Minimum Term. Any such payment must be made to us within 7 days of cancellation. 

 

PAYMENTS 

 3.1      Payments of your membership fees shall be by way of direct debit.  

 3.2      Accordingly, you will be required to sign a direct debit mandate. All direct debit payments will be made to Martin Steels (Emsworth) Limited via Optix Software Limited and are protected by the Direct Debit Guarantee.  

3.3      In the event of the Direct Debit not being paid within 7 days after the due date, we shall suspend the provision of products and services. 

3.4      We shall need to update your membership fee and your Direct Debit to reflect changes such as service improvements, added benefits and inflation. However, we shall seek to give you at least one months’ notice where we need to do so. 

 

DATA

 

4.1     We shall handle any personal data that you may provide to us under this Agreement in accordance with the Data Protection Act 2018 and the General Data Protection Regulation (UK GDPR).  

MISCELLANEOUS 

5.1     This Agreement is exclusive to you and is not transferable. 

5.2      No variations of this Agreement shall be valid unless agreed in writing by us. 

5.3      This Agreement constitutes the entire agreement between the parties and supersedes and extinguishes all previous agreements, promises, assurances, warranties, representations and understandings between them, whether written or oral, relating to its subject matter. 

5.4    We, Martin Steels (Emsworth) Ltd, reserve the right to withhold benefits outlined in 1.2 if we observe evidence of abuse of benefit. 

 

JURISDICTION AND CHOICE OF LAW 

These conditions are governed solely by English Law. Any disputes concerning or arising out of this agreement shall be determined by the courts of England and Wales, which shall have exclusive jurisdiction.
